Project Description
We the People Community Health Worker Corps & Coalition has set out to break barriers to health access by supporting those most impacted by health disparity and closing gaps between healthcare providers, patient care systems and persons in community. As the role of community health workers within health service systems expands, We the People CHWC is leveraging the lived experiences of those who have been forced to navigate the inequities of health access to survive by training them as community health workers and positioning them as bridges to provider care teams and networks. Technology and policy alone can only get us a portion of the way, to fully realize person centered care, We the People CHWC believes we must position people at the center of the formula.
